How To Order Tests - 4 Simple Steps

Fill Out Request Form
If you are interested in ordering a kit for your custom lab panels, please indicate it in the Message field on the left and a CMB Representative will process your order. You will receive an authorizing email or telephone call from a CMB advisor to answer your questions.
Visit Test Center
You will receive an authorizing e-mail confirming your order, payment and shipping address. Included in your kit will be detailed instructions and a test
requisition. You'll need this form when you go to the lab or your physician to get your blood drawn for your test.
Mail Kit To CMB Labs
Once your blood sample is drawn at a local draw station or your physician's office, follow the instructions inside your kit to mail the specimen to CMB Laboratory, a nationally accredited laboratory
for analysis.
Lab Analysis of Results
Once the laboratory receives your specimen kit, your results will be available in one week and sent to your physician.
